Report Submission Form

The electronic version of this completed Final Report should be submitted in Microsoft Word format no later than Tuesday, March 16, 2021.

Note: Financial Summary tables must be completed using the Excel template, which is provided to the project contact via email. Please note that the template is set up to automatically calculate Totals. Completed Financial Summary tables must be submitted in Excel format, as a separate file, along with your completed Final Report.

When completing the report, please use the 2020-2021 Grant Program Guidance Document. As required by the Contribution Agreement with Environment and Climate Change Canada, this Guidance Document was developed to assist Wildlife Habitat Canada grantees in collecting and reporting project data.

If you are submitting more than one Report, please submit each Report separately with the appropriate attachments.

**Do not include more than one Report in the same submission.**

    Always quoting your WHC Project Number in the Subject Line: 21-XXX - Final Report Additional Documentation
    1. Please email Invoice #3, to grants@whc.org, quoting your project number (Microsoft Word, PDF, Excel or JPEG).

    2. If applicable, please email grants@whc.org 1-5 project photos in JPG / JPEG / TIF format to accompany your Report.

    Notes:
    Please quote your project number in the Subject Line
    The upload feature on this website does not always work, so we are doing direct e-mail for any photos.
    We would like to use these photos in good news stories, social media outreach material and inclusion in our annual report.

    3. Please email any additional documents to accompany your Report as one file to grants@whc.org, quoting your project number in the Subject Line.

    Examples such as:
    Copies of Presentations, reports, newsletters, news articles, social media posts, products or marketing tools, areas where you have acknowledged Wildlife Habitat Canada and/or the Habitat Conservation Stamp and the contributions of migratory bird hunters.

    Following the successful submission of your Report and documents, you will receive a confirmation email from Wildlife Habitat Canada indicating that your documents have been received.

    **If your report is not submitting, try removing any uploaded photos (and send them by email instead).
